These Terms of Use and Patient Service Terms govern your use of TheUrgentCareConcierge.com, the Urgent Care Concierge website, online scheduling, Quick Visit Check, patient portal, telehealth services, and related communications.
Urgent Care Concierge is a telehealth service of The Care Concierge, PLLC. The Care Concierge, PLLC d/b/a Urgent Care Concierge provides cash-pay telehealth visits for adults physically located in Arizona at the time of the visit.
By using this website, clicking Start Virtual Visit, completing the Quick Visit Check, scheduling a visit, signing onboarding documents, or receiving services, you agree to these terms.
Not an Emergency Service
Urgent Care Concierge is not an emergency service.
Do not use this website, email, scheduling system, patient portal, or telehealth service for emergencies.
Website Information Is Not Medical Advice
The information on this website is for general informational purposes only. It is not medical advice, diagnosis, treatment, or a substitute for professional medical care.
Use of this website does not establish a patient-provider relationship.
A patient-provider relationship is established only after you complete required onboarding, provide required consent, are accepted for a visit, and participate in a telehealth visit with a medical provider.
Eligibility for Services
Urgent Care Concierge currently provides telehealth visits only for:
- Adults 18 years of age or older
- Patients physically located in Arizona at the time of the telehealth visit
- Low-acuity urgent care concerns that may be appropriate for telehealth
- Patients who complete required forms, consent, and payment
- Patients who understand and agree that the clinician may recommend in-person urgent care, emergency care, labs, imaging, or another level of care if telehealth is not appropriate
We may decline or stop a visit if the concern is not appropriate for telehealth.
Scope of Services
Urgent Care Concierge provides focused telehealth evaluation for common minor illnesses and low-acuity urgent care concerns. Examples may include and are not limited to cold symptoms, cough, sinus symptoms, sore throat, allergies, rash, pink eye or minor eye irritation, UTI symptoms, yeast infection symptoms, nausea, vomiting, diarrhea, heartburn, minor aches, medication questions, and work or school notes when medically appropriate.
The clinician determines whether telehealth evaluation, treatment, prescriptions, labs, imaging, work notes, or referral to another level of care are appropriate.
Services Not Provided
Urgent Care Concierge does not provide emergency care.
Urgent Care Concierge does not provide care for conditions requiring immediate in-person evaluation, procedures, advanced testing, or emergency treatment.
Urgent Care Concierge does not prescribe controlled substances through this telehealth urgent care service. This includes opioid pain medications, benzodiazepines, stimulants, controlled cough medications, sedatives, testosterone, and similar restricted medications.
Urgent Care Concierge does not initiate injectable weight-loss medications through this urgent care telehealth service.
Urgent Care Concierge does not guarantee antibiotics, prescriptions, labs, imaging, work notes, school notes, referrals, or any specific treatment.
Medical Provider Judgment
All care decisions are based on medical provider judgment. The provider may determine that your concern is not appropriate for telehealth, even if you passed the Quick Visit Check or scheduled a visit.
You agree that the provider may recommend in-person urgent care, emergency evaluation, labs, imaging, specialist evaluation, primary care follow-up, or another level of care when clinically appropriate.
Quick Visit Check
The Quick Visit Check is a brief website-based step before scheduling. It helps identify obvious reasons a virtual visit may not be appropriate.
The Quick Visit Check is not a diagnosis, medical evaluation, or guarantee of treatment.
The Quick Visit Check does not collect personal identifying information on the website.
The full intake, patient profile, medical history, pharmacy information, consent forms, scheduling, and chart documentation occur through CharmHealth or another clinical system.
Patient Responsibilities
You agree to:
- Provide accurate and complete information
- Be physically located in Arizona during the visit
- Be available at the scheduled time
- Use a device with appropriate audio/video capability when required
- Provide a safe, private location for the telehealth visit
- Not record the visit without prior written permission
- Not request controlled substances through this service
- Seek emergency care when symptoms are severe, worsening, or potentially life-threatening
- Follow the care plan or seek the recommended level of care
- Contact the appropriate pharmacy, lab, imaging center, urgent care, emergency department, or specialist for outside services when needed
- Pay all applicable fees before or at the time required
Payment
Urgent Care Concierge is a cash-pay service. We do not bill insurance for these virtual urgent care visits.
Payment is due as required before or at the time of scheduling or service.
Outside costs are separate and may include medications, pharmacy charges, lab testing, imaging, in-person urgent care, emergency department care, specialist care, procedures, or other services.
No Guarantee of Outcome
Medicine involves uncertainty. We do not guarantee that telehealth will resolve your concern, that a prescription will be issued, that antibiotics will be prescribed, that a work or school note will be provided, or that you will not need in-person care.
Technology Limitations
Telehealth depends on internet access, device capability, software, patient participation, and third-party platforms. Technical issues may occur. If a technical issue prevents safe or effective care, the visit may need to be rescheduled or you may be directed to in-person care.
Website Availability
We may modify, suspend, or discontinue any part of the website or service at any time without notice.
Intellectual Property
The content, branding, text, graphics, design, logo, and materials on this website belong to The Care Concierge, PLLC or are used with permission. You may not copy, reproduce, sell, or use website content for commercial purposes without written permission.
Prohibited Uses
You may not use this website or service to:
- Seek emergency care
- Submit false or misleading information
- Harass, threaten, or abuse staff
- Attempt to obtain controlled substances or restricted medications through deception
- Interfere with website function or security
- Copy or misuse website content
- Violate applicable law
Third-Party Services
We may use third-party services such as CharmHealth, payment processors, telehealth platforms, hosting providers, email services, or other vendors. Your use of those services may be subject to their own terms and privacy practices.
Limitation of Website Liability
The website is provided on an “as available” basis. To the fullest extent permitted by law, The Care Concierge, PLLC and Urgent Care Concierge are not responsible for website interruptions, technical failures, third-party platform issues, or damages resulting from misuse of the website.
Nothing in these terms is intended to limit rights or responsibilities that cannot be limited under applicable law.
Changes to These Terms
We may update these Terms of Use and Patient Service Terms at any time. The updated version will be posted on the website with an updated effective date.
Patient Agreement
By scheduling a visit, signing onboarding forms, or receiving services from Urgent Care Concierge, you acknowledge and agree that:
- You have reviewed these Terms of Use and Patient Service Terms
- You understand Urgent Care Concierge is not an emergency service
- You understand services are limited to adults physically located in Arizona
- You understand telehealth may not be appropriate for your concern
- You understand controlled substances are not prescribed through this service
- You understand prescriptions, antibiotics, labs, imaging, and work or school notes are not guaranteed
- You understand outside costs are separate
- You agree to the payment and refund policies
